The short answer

Symptoms may fluctuate and can coexist with psoriasis, nail changes, swollen peripheral joints or painful tendon attachments. Their pattern helps guide assessment but cannot confirm the condition on its own.

Patterns worth noticing

Pain may be felt low in the back, deep in one or both buttocks, or higher along the spine. Inflammatory stiffness often becomes noticeable after sleep or prolonged sitting, and some people find gentle movement more comfortable than continued rest. Pain can interrupt sleep or make turning in bed difficult.

Stay gently active within tolerable limits while waiting for review, using shorter bouts if a long session aggravates symptoms. Avoid starting or stopping prescription anti-inflammatory or immune-modifying medicine without clinical guidance. An examination may cover spinal movement, sacroiliac areas, peripheral joints, skin, nails and tendon attachments, with tests chosen according to the findings.

Symptoms that should not wait

Seek emergency care for back pain accompanied by new weakness in the legs, loss of bladder or bowel control, numbness around the groin, collapse, serious trauma, or fever with rapidly worsening illness. These are red flags regardless of whether psoriatic arthritis is already diagnosed.

Obtain same-day eye assessment for a painful red eye, marked light sensitivity or blurred vision. A hot, intensely painful swollen joint, or possible infection while taking medicine that alters immune activity, also needs rapid medical advice.

Ask whether your pain pattern suggests inflammation or a mechanical cause, what other diagnoses are being considered, and which tests would influence treatment. Discuss how to recognise a flare, what movement plan suits your current symptoms and which medication side effects warrant contact.
