Asthma and hoarseness: what to know

Asthma mainly affects the lower airways, while hoarseness comes from altered vibration or irritation around the vocal cords. The two can occur at the same time without having the same cause.

Repeated coughing and throat clearing can strain the voice. A cold or laryngitis may inflame the voice box, and acid reflux can irritate the throat even without obvious heartburn. Some preventer inhalers can also contribute to local throat irritation or a fungal mouth infection, particularly when medicine remains in the mouth and throat.

A clinician may review when the change began, whether it follows inhaler use, and whether you also have swallowing difficulty, neck swelling, pain, fever, or an ongoing cough. These details help separate voice-box irritation from an asthma flare or another problem.

Keep taking prescribed asthma medicine unless the prescriber tells you to change it. Follow your asthma action plan, use the inhaler technique you were taught, and rinse your mouth and spit after a steroid preventer inhaler. A spacer may reduce medicine left in the mouth when it is suitable for your inhaler.

Rest your voice without whispering, sip fluids, and avoid smoke or vaping. Arrange a medical review if hoarseness persists, keeps returning, or began after a treatment change; do not stop a preventer on your own because poorly controlled asthma can be dangerous.

Seek emergency help for severe breathlessness, difficulty speaking because you cannot get enough air, blue or grey lips, confusion, collapse, or asthma symptoms that do not improve after using your reliever as directed in your action plan. Sudden throat swelling, noisy breathing, or trouble swallowing saliva also needs emergency assessment.

Book a prompt appointment for coughing up blood, a new neck lump, painful swallowing, unexplained weight loss, or a voice change that does not settle. These signs require examination but do not establish a diagnosis by themselves.

Ask whether your inhaler technique could be irritating your throat, whether a spacer fits your device, and whether your mouth and throat should be checked for infection. You can also ask if reflux, persistent coughing, allergy, or a voice-box problem fits the timing of your symptoms and whether an ear, nose and throat assessment is appropriate.
