What should you know about Aspergillus osteomyelitis?

It can involve the spine or another bone and may develop when infection spreads through the bloodstream or directly from nearby tissue or a procedure. Care often brings together infectious-disease, orthopaedic, microbiology and radiology teams.

Persistent focal bone pain is an important presentation; spinal involvement may also cause back pain. Fever may be present or absent. Risk can be higher when immune defences are weakened, but the infection cannot be confirmed from symptoms or risk factors alone.

Imaging helps locate abnormal bone and surrounding tissue, while blood tests can look for inflammation or evidence of fungal infection. A tissue or bone sample for microscopy, culture and pathology may be needed to identify the organism and distinguish infection from a tumour or another inflammatory process.

Treatment uses a prescribed systemic antifungal selected for the organism, infection site, other medicines and the patient’s kidney and liver health. Therapy may be prolonged, and scheduled blood tests or drug-level checks can help the team manage interactions and side effects.

Surgery may be considered to obtain diagnostic tissue, drain a collection, remove infected or unstable tissue, relieve pressure on nerves, or stabilize damaged bone. Do not stop antifungal medicine or change the dose without the treating team, even if pain improves.

Seek urgent medical care for new limb weakness, spreading numbness, difficulty walking, loss of bladder or bowel control, or numbness around the saddle area. With spinal infection, these changes may indicate pressure on neural structures.

Prompt review is also needed for rapidly worsening pain, fever with feeling acutely unwell, wound drainage, confusion, breathing difficulty or a severe reaction after medication. A person already receiving treatment should use the team’s urgent contact route for new jaundice, persistent vomiting or other suspected medicine toxicity.

Ask which sample confirmed Aspergillus, whether susceptibility testing is available, which bone is affected and whether there is instability or nerve compression. Clarify how response and medicine safety will be monitored, which drug interactions matter, what activity is safe and what would make surgery necessary.
