ASOT blood test: purpose and interpretation

ASOT is also called the ASO titre. It is usually considered when symptoms arising after a throat or skin infection raise concern about an immune-related complication. It is not a general screening test and cannot locate an infection.

A higher titre indicates that the immune system has encountered streptolysin O recently enough for antibodies to remain present. It does not establish that bacteria are still active, and a raised value can occur in someone who has recovered and has no complication.

A lower result may reflect no recent exposure, an early sample or a weaker antibody response. Interpretation depends on the laboratory's reference interval, age, symptom timing and results from examination, urine testing or other blood work. A trend across paired samples may offer useful context.

Unless the clinician is combining it with a test that requires fasting, you can usually eat and drink normally. Mention any recent antibiotics, sore throat, skin sores, fever, rash, joint symptoms, urine changes or swelling, even if the original illness seemed minor.

Review the report with the clinician who knows why it was requested. A positive or raised titre alone is not a reason to self-treat with leftover antibiotics. Follow-up should address the current symptoms and the organ system involved rather than chasing the antibody number.

Get prompt medical help for dark or bloody urine, much less urine than usual, facial or leg swelling, shortness of breath, chest discomfort, fainting, severe joint inflammation, persistent fever or new involuntary movements. The urgency comes from these symptoms, not from whether an ASOT result is high.

The needle site normally needs only simple pressure and a small dressing. Seek advice if bleeding will not stop or if spreading redness, increasing warmth, marked swelling or discharge develops.

Find out which suspected problem the test is helping assess, whether the sample timing is suitable, what accompanying tests show, whether another titre is needed and which changes in your symptoms should trigger earlier review.
