How heart failure can lead to ascites

When the heart cannot move blood forward effectively, pressure may rise behind it. Pressure in the veins serving the liver and abdomen can push fluid into the abdominal cavity. Heart failure that affects the right side of the heart is particularly associated with swelling in the abdomen, legs and ankles.

Ascites may make the abdomen enlarge, feel tight or become uncomfortable. It can reduce appetite and make breathing harder by limiting movement of the diaphragm. Heart failure may also present with breathlessness, fatigue, cough or rapid weight gain from retained fluid.

Contact your heart-care clinician promptly if abdominal size, ankle swelling, breathing or weight changes. Keep taking prescribed heart medicines unless a clinician tells you otherwise, and do not double a water-tablet dose on your own.

A review may include examination of the heart, lungs, abdomen and legs, along with blood tests and imaging. The clinician will look for the cause of the fluid and check kidney function and salts before adjusting treatment. Follow the personal fluid and salt advice already given by your care team, since the right limits differ between patients.

Call emergency services for severe breathing difficulty, chest pressure, fainting, blue or grey lips, new confusion, or breathlessness that prevents speaking comfortably. Seek same-day assessment for fever with abdominal pain, a rapidly enlarging abdomen, much less urine, or sudden worsening swelling.

Is every swollen abdomen in heart failure caused by ascites?

No. Bloating, bowel problems, body-weight changes and other conditions can also enlarge the abdomen, so an examination is needed to distinguish them.

Can I take extra diuretic medicine when my abdomen swells?

Only follow a dose-adjustment plan that your clinician has specifically provided. Extra diuretic can disturb kidney function, blood pressure and body salts.
