Asacol for ulcerative colitis: uses and precautions

Take the prescribed formulation exactly as directed because different mesalazine brands and tablet strengths may release medicine in different parts of the bowel. Do not substitute products or stop treatment simply because symptoms improve without checking with the prescriber.

Ulcerative colitis causes inflammation in the lining of the large bowel. Mesalazine acts locally in the bowel to reduce that inflammation. It may be prescribed during a flare and continued when symptoms settle to reduce the chance of inflammation returning.

Response is judged from changes in diarrhoea, bleeding, urgency, abdominal pain, general wellbeing, and sometimes blood, stool, or bowel investigations. The medicine may not relieve every symptom immediately, and worsening symptoms can reflect an active flare, infection, or another problem requiring review.

Follow the label about swallowing, food, and handling the tablet; modified-release tablets generally should not be crushed unless the pharmacist confirms this is appropriate for that product. Keep hydrated, particularly during vomiting, diarrhoea, fever, or hot weather. If a dose is missed, follow the leaflet or pharmacist's advice rather than doubling the next dose.

Your clinician may arrange kidney function and other blood tests before and during treatment. Share your full medicine list and mention kidney or liver problems, previous reactions to mesalazine or aspirin-like medicines, and pregnancy or breastfeeding. Contact the prescriber if intact tablets repeatedly appear in the stool.

Seek urgent care for facial or throat swelling, breathing difficulty, a severe blistering rash, chest pain, marked weakness, severe abdominal pain, heavy rectal bleeding, black stools, fainting, or inability to keep fluids down. Fever with worsening bowel symptoms also warrants prompt clinical advice.

Contact the prescribing team promptly for unexplained bruising, persistent sore throat, reduced urination, blood in the urine, yellowing of the skin or eyes, or a new severe headache with neck stiffness. These symptoms do not establish a medicine reaction, but they require assessment.

Ask how long to continue Asacol, how you will know it is working, which monitoring tests are planned, and whom to contact during a flare. Clarify what to do during dehydration, whether other medicines interact with it, and whether your ulcerative colitis changes the timing of bowel surveillance.
