Arthritis and Crohn's disease: what to know

This is often called enteropathic arthritis. It can affect larger joints in the arms or legs, the spine, or the joints linking the spine and pelvis. Joint symptoms may rise and fall with intestinal inflammation, but some patterns follow a separate course. A clinician should distinguish inflammatory arthritis from injury, infection, medicine effects and other joint conditions.

Inflammatory joint symptoms can include swelling, warmth, stiffness after rest and pain that improves as you move. Back or buttock pain may be worse after inactivity and ease with gentle movement. Crohn's disease itself more often presents with bowel features such as diarrhoea, abdominal pain, urgency, tiredness or weight loss; the presence or absence of these symptoms does not by itself explain a painful joint.

Assessment may include the timing and pattern of symptoms, examination of the joints and abdomen, and blood tests for inflammation, anaemia or medicine safety. Imaging or specialist assessment may be appropriate when the spine is involved, swelling persists or the diagnosis remains uncertain.

Tell your gastroenterology or primary-care team about new joint pain, morning stiffness, swelling, eye symptoms and changes in bowel activity. Coordinating care matters because a treatment chosen for Crohn's disease may also help certain joint problems, while another medicine may target the joints specifically. Gentle movement and physiotherapy can help maintain mobility when tailored to the affected area.

Do not start regular anti-inflammatory painkillers such as ibuprofen without checking with a clinician who knows your Crohn's history, as these medicines can aggravate digestive symptoms in some people. Do not stop prescribed Crohn's or arthritis treatment suddenly; report side effects so the plan can be adjusted safely.

Seek urgent assessment for a single joint that becomes suddenly hot, very swollen and difficult to move, particularly with fever or feeling severely unwell. Joint infection and other acute causes need prompt evaluation. Emergency care is also appropriate for severe abdominal pain with a rigid or markedly swollen abdomen, repeated vomiting, fainting, heavy rectal bleeding or inability to keep fluids down.

Contact your care team promptly for worsening diarrhoea, blood in the stool, persistent joint swelling, new eye pain or redness, or symptoms that prevent walking or normal daily tasks. Eye pain with light sensitivity or blurred vision needs same-day assessment because inflammation can threaten sight.

Ask whether the pattern looks inflammatory, whether bowel and joint activity appear linked, and which pain relief is suitable for you. Discuss whether blood tests, imaging, physiotherapy or referral to rheumatology is needed, and confirm which signs should prompt contact with your gastroenterology team.
