This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

The short answer

Armpit sweat is naturally odourless, but skin bacteria can break it down and create a noticeable smell. Heat, exercise, stress, clothing and hormonal changes can affect sweating or make the odour easier to notice. A change in smell alone does not identify a specific illness.

Why smell and sweating happen

The underarms contain sweat glands that become active with warmth and emotion. Moisture mixing with bacteria on the skin, hair and clothing produces much of the familiar underarm odour. Spicy foods, alcohol, smoking, some medicines and an infection or irritated skin can also alter the pattern. Excessive sweating that occurs beyond what is needed for cooling may be hyperhidrosis, especially when it soaks clothing or disrupts daily activities.

Practical ways to manage it

Wash the underarms gently, dry them well and change damp clothing promptly. Breathable fabrics and rotating freshly washed tops can reduce trapped moisture and odour. A deodorant mainly controls smell, while an antiperspirant reduces sweat. Apply antiperspirant to completely dry, unbroken skin as directed on its label; using it at night can help it settle into the sweat ducts. Stop if significant burning or a rash develops.

If non-prescription products are not enough, a clinician can assess triggers and discuss stronger topical treatment. Prescription antiperspirant may be considered first, while botulinum toxin injections are an option for suitable people with troublesome underarm sweating. The choice depends on skin sensitivity, medical history, medicines and how much sweating affects everyday life.

When to seek medical care

Arrange a medical review if sweating starts suddenly without a clear trigger, repeatedly wakes you at night, affects your whole body, or comes with a persistent rash, painful lump, drainage or broken skin. Prompt care is appropriate when heavy sweating occurs with chest pain, faintness, severe breathlessness, confusion or marked weakness. A clinician may review medicines and look for infection, thyroid problems, blood-sugar changes or other causes based on your symptoms.

Questions for your clinician

Ask whether your pattern fits hyperhidrosis, whether a medicine or skin condition could be contributing, and which antiperspirant strength suits your skin. It can also help to discuss what to do if irritation occurs and whether an injectable treatment is appropriate for your goals and health history.

Questions people ask

Why do my armpits smell even after washing?

Odour can return as sweat, skin bacteria and residue in clothing build up again. Drying thoroughly, changing damp tops and using a suitable deodorant or antiperspirant may help. Persistent odour with a rash, pain or drainage deserves assessment.

Is deodorant the same as antiperspirant?

No. Deodorant is designed mainly to reduce or mask odour, whereas antiperspirant limits sweat reaching the skin. Some products combine both actions.

Can stress make underarm sweating worse?

Yes. Emotional signals can activate underarm sweat glands, so anxiety, excitement or pressure may cause a sudden increase even when the environment is cool.
