Armpit compression garments: coverage, comfort, and safety

Standard arm sleeves may stop below the axilla and may not address swelling in the armpit or side of the chest. Depending on the area involved, a clinician may suggest a garment with a shoulder section, chest panel, or another made-to-measure design.

The axilla moves and folds whenever the shoulder or arm moves, so compression must remain in contact without cutting into a crease. This region can also be sensitive after surgery or radiotherapy, and moisture can increase friction. Effective coverage depends on the exact location of swelling: the upper arm, hollow of the armpit, side of the chest, or a combination. A generic sleeve cannot always apply useful pressure across all of these contours.

Ask a lymphoedema clinician or trained fitter to inspect the garment while your arm rests at your side and while you reach forward and overhead. The fabric should not bunch, expose the swollen area, pull across a scar, or restrict shoulder movement. Keep the skin clean and dry, check the crease for redness or broken areas, and follow product guidance about washing. Avoid placing improvised padding in the armpit because it can shift and produce uneven pressure.

Arrange prompt assessment for new or rapidly increasing armpit, arm, breast, or chest-wall swelling; spreading redness or warmth; fever; drainage; an opening wound; or pain that is escalating. Discontinue the garment and seek advice if it causes numbness, hand colour change, a cold hand, blistering, or a deep pressure line that does not settle. Sudden difficulty breathing, chest pain, or collapse requires urgent medical help.

Show the clinician exactly where fullness or tightness occurs and ask which design can maintain contact there during movement. Discuss how scars, radiotherapy-related skin sensitivity, breast or chest-wall swelling, and your usual clothing affect the choice. Request clear instructions for wear, skin products, cleaning, refitting, and what to do if the garment slides away from the armpit.
