Arm physiotherapy exercises: a safe starting framework

Short answer
Arm physiotherapy exercises should match the affected tissue, current movement, strength, symptoms, and personal recovery goals.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Programme focus
- Exercises may target mobility, strength, coordination, nerve tolerance, or a specific task.
- Progression
- Change range, load, repetitions, or complexity gradually rather than all together.
- Stop signal
- New weakness, numbness, or spreading tingling needs reassessment.
The key principle
A useful programme may include gentle range-of-motion work, controlled resistance, coordination, and practice of a relevant task. The right choice differs for a stiff shoulder, an irritated tendon, a healing injury, or nerve-related arm symptoms.
Why exercise selection is individual
The arm works as a chain: the neck and shoulder position the limb, the elbow changes reach, and the wrist and hand control load. An exercise can help one part while overloading another if technique, resistance, or range is poorly matched. Assessment identifies whether the immediate priority is mobility, muscle capacity, tendon loading, nerve tolerance, or movement control.
Some discomfort during rehabilitation can be acceptable, but sharp pain, spreading tingling, new numbness, or loss of strength is a reason to stop and reassess. The response later that day and the next morning also helps determine whether the dose was manageable.
A sensible exercise sequence
Begin with comfortable movements such as opening and closing the hand, bending and straightening the elbow, rotating the palm up and down, or supported shoulder motion. Use only the movements that fit your condition and any restrictions from your clinician. Move smoothly and avoid holding your breath.
When basic movement is tolerated, resistance can be introduced with an isometric hold, a light band, or a small load. Change one variable at a time: range, resistance, repetitions, or task difficulty. A physiotherapist can show the target position, define an acceptable symptom response, and adapt the programme to work, sport, sleep, and daily activities.
When not to push through
Pause the programme and arrange prompt assessment for new or increasing weakness, persistent numbness, marked swelling, redness and warmth, fever, or pain that is escalating rather than settling. After a fall or direct impact, deformity or inability to use the arm needs urgent evaluation.
Seek emergency help if arm discomfort is accompanied by chest pressure, breathlessness, sweating, faintness, or nausea. Sudden loss of power, facial droop, or difficulty speaking also requires emergency services. These features are not appropriate to manage by trying a different exercise.
What your programme should explain
Ask what each exercise is intended to change, where you should feel it, which sensations mean stop, and how progression will be decided. Confirm whether any movement is temporarily restricted and bring up neck symptoms, hand tingling, previous surgery, recent trauma, or medical conditions that may alter exercise selection.

Questions people ask
Should arm exercises hurt?
They should not cause sharp or escalating pain. A clinician may permit mild local discomfort for some conditions, provided it settles and does not bring weakness, numbness, or spreading symptoms.
Can I copy someone else's arm exercise programme?
That may load the wrong tissue or use an unsuitable range. Exercise choice should reflect your diagnosis or working assessment, current capacity, and functional goal.
How do I know when to add resistance?
Add resistance when the current movement is controlled and produces an acceptable response during and after exercise, following the progression agreed with your physiotherapist.
