The short answer
It can occur across body sizes and is not an infection. Skin laxity, fat distribution, connective-tissue structure, genetics, and ageing can all influence how noticeable it looks.
Why the surface looks uneven
Fibrous bands anchor the skin to deeper tissue while fat pushes between them, producing shallow depressions and raised areas. Loose skin may accentuate that contrast, which is why a treatment that addresses laxity may not release a distinct tethered dimple.
Cellulite does not itself cause fever, spreading redness, or sudden one-sided swelling. Those findings require evaluation for a medical problem rather than cosmetic cellulite care.

No treatment is medically required. If the texture bothers you, begin with an examination that separates discrete tethered dimples from general laxity and fat prominence. Stable weight, regular movement, moisturiser, and sun protection can support skin health but may not remove structural dimpling.
Options such as radiofrequency skin tightening, RF microneedling, carboxytherapy, or subcision act differently. Ask for a plan matched to the dominant feature, along with expected downtime, aftercare, the chance of bruising or pigment change, and the likelihood of needing maintenance.
When the change may not be cellulite
Arrange medical assessment for a new lump, persistent pain, firm skin, marked asymmetry, or a rapid change in one arm. Seek prompt care when swelling develops suddenly or is accompanied by redness, heat, fever, breathlessness, chest discomfort, or weakness.
After a cosmetic procedure, contact the treating clinician about worsening pain, blistering, drainage, spreading redness, or altered sensation that is not settling.
