Notice whether discomfort begins under the big toe, in a smaller toe, along the inner arch or across the whole forefoot. Also track swelling, redness, stiffness, footwear pressure and whether pain appears during push-off.

What the pattern may mean

A bunion can present with a prominent, tender big-toe joint and may change how pressure travels through the arch. Hallux rigidus can present with big-toe stiffness and pain during push-off. Hammer toe can present with a bent smaller toe that rubs against footwear.

Gout often presents as sudden, intense joint pain with warmth, redness and swelling, commonly at the big toe. Pain without these features may instead reflect footwear compression, a change in activity or irritation of muscles, tendons and joints. An examination is needed to distinguish overlapping patterns.

What you can do now

Reduce activities that reproduce the pain, but keep comfortable movement if you can walk normally. Choose shoes with enough toe-box width, a stable sole and no pressure on a tender prominence. A wrapped cold pack may calm a sore or swollen area; protect the skin and use short applications.

Do not force a stiff or swollen toe through painful stretches. When symptoms settle, resume distance and impact gradually. Record which shoes and movements trigger pain because that detail can help a clinician assess loading and joint motion.

When to seek medical care

Seek urgent assessment after a significant injury if the foot looks deformed, you cannot bear weight, a toe becomes pale or blue, or numbness develops. Sudden severe pain with a hot, red, swollen joint also warrants prompt care, especially with fever or feeling unwell.

Arrange a routine appointment if pain keeps returning, disrupts walking, or does not improve with activity adjustment and suitable footwear. People with diabetes, reduced sensation, poor circulation or a foot wound should seek earlier clinical advice.
