The short answer
ARBs are medicines commonly used for blood pressure or heart and kidney conditions. A reaction can occur even when the medicine was previously tolerated, so a new episode should not be dismissed solely because treatment is not new.
How the reaction may appear
Angioedema presents as swelling beneath the skin or lining of the mouth. In a medicine-associated episode, it may affect the lips, face, tongue, mouth, or throat. The skin may look normal or feel tight, and itching or hives may be absent.
A clinician considers the timing, medicine list, previous reactions, and other possible causes. ARB association cannot be confirmed from appearance alone; foods, other medicines, allergy, infection, and inherited or acquired swelling disorders can produce a similar pattern.
What to do after swelling starts
If an ARB may be responsible, seek prompt medical advice and do not take another dose until the prescribing clinician or emergency team has assessed the reaction. Take the medicine packet or an accurate medication list with you. Do not restart the suspected drug later unless a clinician who knows about the episode specifically instructs you to do so.
Treatment depends on the swelling mechanism and airway risk. Antihistamines and corticosteroids can help some allergy-related episodes but may have little effect when swelling is not driven by histamine. Airway assessment and observation can therefore matter more than whether a rash is present.
When emergency help is needed
Call emergency services now for tongue or throat swelling, trouble breathing, noisy breathing, difficulty swallowing saliva, drooling, a changed or muffled voice, faintness, or swelling that is progressing quickly. Do not drive yourself if breathing or consciousness may worsen.
Swelling limited to the face or lips still deserves urgent same-day assessment because it can extend toward the airway. Improvement at home does not establish the cause; the blood-pressure or heart treatment plan also needs review before another ARB dose is considered.

Ask which medicine is suspected, whether it should be recorded as a serious drug reaction, and what alternative will manage the condition for which the ARB was prescribed. Mention any earlier swelling with an ACE inhibitor, ARB, food, or unexplained trigger, and ask for a written plan in case swelling returns.
