Using an ARB for hypertension

The choice of an angiotensin II receptor blocker depends on other health conditions, kidney function, potassium, current medicines and previous treatment. It may be prescribed alone or alongside another blood-pressure medicine.

Hypertension is diagnosed from properly taken readings over time, often including home or ambulatory measurements. A single high result during pain, stress or illness may need confirmation before it represents a person's usual pressure.

Once treatment begins, the aim is sustained control while avoiding dizziness, kidney problems and abnormal potassium. The target is individualised according to age, frailty, cardiovascular risk and other conditions. Feeling well does not show whether pressure is controlled, so measurements remain necessary.

Use the ARB on the prescribed schedule and record home readings with the date, time and any symptoms. Sit quietly before measuring, support the arm and use a cuff that fits. Bring the monitor or record to appointments so patterns, rather than isolated readings, can guide decisions.

Reducing excess dietary salt, staying active within your ability, moderating alcohol, stopping smoking and addressing sleep apnoea or excess weight can support blood-pressure control. These measures complement prescribed treatment; they do not justify stopping it without a review.

Seek immediate help for chest pain, severe breathlessness, new weakness or numbness on one side, facial droop, trouble speaking, sudden confusion, collapse or a sudden severe headache. These symptoms require urgent assessment whether or not a blood-pressure reading is available.

Contact your clinician for repeated readings outside the range they set for you, frequent light-headedness, fainting, reduced urine, muscle weakness or persistent vomiting or diarrhoea. Illness and dehydration can change how an ARB affects blood pressure and kidney function.

Ask what target range applies to you, how often to record readings, when blood tests are due and which result should prompt a call. Clarify the plan for missed doses, dehydration, pregnancy planning and over-the-counter medicines that may raise pressure or strain the kidneys.
