Appendicitis and black stool

Appendicitis more often presents with worsening abdominal pain, commonly moving toward the lower right side, plus nausea, reduced appetite, vomiting, or fever. Black stool and suspected appendicitis can still occur together, so one symptom should not be used to explain away the other.

Bleeding from the stomach or upper intestine can be digested during passage through the bowel, producing stool that is very dark, sticky, tar-like, and unusually strong-smelling. Peptic ulcer and gastritis can present with this type of bleeding, and Helicobacter pylori infection can contribute to ulcer disease.

Iron supplements, bismuth-containing medicines, and some dark foods can also darken stool. Colour alone cannot reliably separate these causes from bleeding, particularly when the stool is tarry or accompanied by illness.

Arrange prompt medical assessment for unexplained black, tarry stool. Note when it began, its consistency, any red blood, abdominal pain location, vomiting, dizziness, and every medicine or supplement taken. Specifically mention aspirin, anti-inflammatory pain relievers, blood-thinning medicine, iron, and bismuth products.

Do not take additional anti-inflammatory pain relievers for unexplained abdominal pain or suspected bleeding unless a clinician has directed you. Avoid food and drink if emergency staff advise this because urgent tests or a procedure may be considered.

Seek emergency help now if black stool occurs with fainting, marked dizziness, weakness, confusion, rapid breathing, severe or worsening abdominal pain, a rigid abdomen, vomiting blood, or vomit resembling coffee grounds. These signs can accompany significant bleeding or an acute abdominal illness.

Also seek urgent assessment for right-sided abdominal pain that intensifies, pain made worse by movement, persistent vomiting, fever, or inability to drink. Appendicitis requires timely clinical evaluation regardless of stool colour.

Ask whether the stool description suggests digested blood, whether blood tests or digestive-tract evaluation are needed, and how your medicines should be handled. Clarify which change in pain, vomiting, dizziness, or stool should send you directly to emergency care.
