Can anxiety cause chest pain?

An episode may feel sharp, tight, aching, or burning and can occur with a racing heart, tingling, dizziness, sweating, or a sense of panic. Similar symptoms can arise from the heart, lungs, chest wall, or digestive tract, so anxiety should not be assumed from sensation alone.

Why anxiety can hurt

During the stress response, chest and shoulder muscles may brace while breathing becomes quicker and shallower. Overbreathing can lower carbon dioxide in the blood temporarily, contributing to tingling, light-headedness, or tightness. Heightened attention to the heartbeat can then amplify discomfort and fear.

Pain that repeatedly follows anxious thoughts and eases with relaxation may fit this pattern. Even so, location or quality cannot reliably exclude angina, a lung problem, reflux, or another physical cause.

What to do during an episode

Stop strenuous activity, sit somewhere safe, and let your shoulders drop. Breathe gently through the nose and allow an unforced, slower breath out; repeated deep breaths can worsen overbreathing. Ground your attention by naming what you can see and feel around you.

If a clinician has already confirmed an anxiety pattern, follow the agreed coping plan. Keeping notes on activity, stress, meals, duration, and accompanying symptoms can help identify triggers. Recurrent episodes may improve with structured psychological therapy, sleep support, regular movement, and prescribed treatment when appropriate.

Chest pain warning signs

Call emergency services for severe, sudden, or persistent chest pain, especially with breathlessness, fainting, cold sweat, vomiting, marked weakness, or pain spreading to the arm, jaw, back, or shoulder. Sudden chest pain with coughing blood, blue lips, or one swollen painful leg also requires emergency assessment.

Seek prompt medical advice for a first episode, pain brought on by exertion, a changing pattern, fever or cough, or symptoms that remain unexplained. Do not drive yourself if you feel faint or severely unwell.
