Antibiotics for perioral dermatitis

A clinician may select a topical medicine for a limited eruption or an oral medicine when bumps are extensive, persistent, or difficult to control. The choice accounts for pregnancy, age, allergies, interactions, previous medicines, and how sensitive the facial skin is.

Perioral dermatitis often causes clusters of small red or skin-coloured bumps around the mouth and may also affect the nose or eyes. Burning, tightness, flaking, and sensitivity can accompany the bumps. The skin immediately beside the lip edge may remain relatively clear.

The prescribed antibiotic is used partly for its anti-inflammatory action. This differs from treating a straightforward skin infection. Topical corticosteroids on the face can trigger or sustain the condition, and abrupt withdrawal after regular use can cause a temporary flare, so a clinician may recommend a planned change.

Use prescribed treatment consistently and keep the routine gentle. Wash with lukewarm water and a mild cleanser if tolerated, then choose a simple fragrance-free moisturiser that does not sting. Pause heavy cosmetics, facial scrubs, fragranced oils, and unnecessary medicated creams while the skin is reactive.

Do not restart a corticosteroid cream to suppress a flare unless the prescriber specifically directs it. Tell the clinician about steroid creams, nasal sprays, inhalers, cosmetics, toothpaste changes, and acne products because these details can shape the plan. Improvement may be gradual rather than immediate.

Arrange review if the eruption is worsening despite treatment, spreading close to the eyes, crusting heavily, becoming very painful, or producing pus. Eye redness, eye pain, light sensitivity, or a change in vision needs prompt assessment. Seek urgent help for facial or tongue swelling, breathing difficulty, faintness, or a widespread blistering rash after taking medicine.

Ask whether the eruption fits perioral dermatitis rather than acne, rosacea, eczema, or infection; which products should be paused; and how to manage any corticosteroid withdrawal flare. Confirm how long to continue the antibiotic, what improvement should look like, and what options are compatible with pregnancy, breastfeeding, or other regular medicines.
