The short answer

Gangrene means body tissue is dying because its blood supply has been lost, an infection is damaging it, or both. Antibiotics cannot revive dead tissue or reopen a blocked artery, so clinicians may need to remove damaged tissue and restore circulation urgently.

Why antibiotics are only one part of care

Hospital teams may start antibiotics through a vein when a serious bacterial infection is suspected, then refine treatment using examination, blood tests, imaging and samples from the affected area. The selected medicines depend on the likely bacteria, the site and the person's medical condition.

Surgery can remove dead or infected tissue and reduce the source of toxins and bacteria. If restricted blood flow is responsible, a vascular procedure may be considered. Pain relief, wound care and treatment of blood sugar or other underlying problems can be needed as well.

What to do if gangrene is suspected

Go to an emergency department now if you think tissue may be gangrenous. Do not wait for a routine appointment, take leftover antibiotics, apply harsh chemicals, or cut the area. Keep the affected part protected from pressure and bring a current medicines list if this does not delay leaving.

Tell the emergency team about diabetes, circulation disease, recent injury or surgery, immune suppression, smoking, medication allergies and when the changes began. These details help the team act on both infection and impaired blood supply.

Emergency warning signs

Black, purple, grey or blistered skin, loss of sensation, severe pain followed by numbness, foul-smelling discharge, crackling beneath the skin or rapidly increasing swelling all require emergency assessment. Call emergency services if the person is confused, faint, breathless, very weak, clammy or difficult to wake.

A rapidly changing wound is urgent even without fever. Reduced sensation from diabetes or nerve damage can make a dangerous foot problem feel less painful than expected.

Ask whether infection, blocked blood flow or both are contributing; what operation or circulation treatment is being considered; how samples will guide antibiotics; and how the wound will be protected afterward. Family members can also ask which changes should be reported immediately during recovery.
