Antibiotics and nappy rash

The rash does not automatically mean the antibiotic must be stopped. Continue it as prescribed unless a clinician advises otherwise, and seek advice if the eruption is severe, spreading, or accompanied by other symptoms.

An irritant rash often affects skin in direct contact with the nappy and may spare deeper creases. A yeast-related rash can look bright red, involve the folds, and have smaller red spots around its edges. Appearance alone is not always enough to identify the cause, particularly if creams have already been used.

Antibiotic-related loose stool increases contact between skin and irritating moisture. Frequent wiping can add friction, while a warm, covered area allows irritation to continue. A separate widespread rash on the trunk, face, or limbs needs a different assessment because it may represent a medicine reaction or another illness.

Change wet or soiled nappies promptly. Clean with lukewarm water and soft cotton or a fragrance-free wipe, then pat dry instead of rubbing. Allow nappy-free time when practical and apply a thick barrier ointment to clean, dry skin at each change.

Avoid fragranced soaps, talc, antiseptics, and tight nappies while the skin heals. Do not apply an antifungal, steroid, or combination cream to a baby without guidance on the right product and duration. Keep offering usual feeds if stools are loose and watch for signs of reduced hydration.

Contact a clinician if the rash is not improving with careful skin protection, extends beyond the nappy area, keeps returning, or has blisters, open sores, pus, crusting, marked swelling, or significant pain. A baby who has fever, feeds poorly, is unusually sleepy, has frequent watery diarrhoea, or produces fewer wet nappies also needs prompt advice.

Get emergency help if there is breathing difficulty, facial or mouth swelling, limpness, or a rapidly spreading rash with the baby appearing very unwell. These findings are not typical of simple nappy irritation.

Tell the clinician when the antibiotic started, when the rash appeared, whether the folds are involved, and whether stools have changed. Ask if the pattern suggests irritation or yeast, which barrier or treatment is suitable for your baby's age, and how long improvement should take.
