The short answer

The combination can occur with viral illnesses as well as strep throat and other bacterial infections. A clinician uses the surrounding symptoms, examination, medical history, and sometimes a throat test to choose treatment.

Interpreting both symptoms

Influenza, COVID-19, glandular fever, and some common colds can present with throat pain and fever. Strep throat commonly presents with fever and painful swallowing, while tonsillitis can be caused by either viruses or bacteria. The presence of fever does not by itself show that an antibiotic is needed.

Features such as tender neck glands, tonsil swelling, rash, cough, nasal symptoms, exposure history, and the person's age help shape the assessment. Testing may be useful when the result would alter care.

Drink small amounts often and monitor urine output, alertness, swallowing, and breathing. Rest and use an age-appropriate fever or pain medicine only after checking its directions and whether it is safe with existing conditions or medicines.

Arrange medical advice if the fever continues, symptoms intensify, a rash appears, or eating and drinking become difficult. When prescribed an antibiotic, follow its label, complete the intended course unless advised otherwise, and never give it to another person.

When not to wait

Emergency assessment is needed for trouble breathing, drooling or inability to swallow saliva, rapidly increasing neck or mouth swelling, severe drowsiness, confusion, a seizure, or a rash that does not fade under pressure. Very little urine, dizziness, or a dry mouth can accompany significant dehydration.

Seek prompt advice for fever and throat symptoms in a person who is immunocompromised or becoming rapidly more unwell. If hives, facial swelling, wheezing, or throat tightness follows an antibiotic dose, treat it as a possible serious allergy.
