Antibiotics for preseptal cellulitis

Treatment choice depends on age, illness severity, the likely source, medicine allergies and local resistance patterns. Some patients can take oral treatment with reliable follow-up, while severe infection, inability to take medicine or diagnostic uncertainty may require hospital assessment and intravenous treatment.

Preseptal cellulitis is an infection of the eyelid and nearby skin in front of the tissue barrier around the eye socket. It commonly presents with one-sided eyelid redness, warmth, tenderness and swelling, while eye movement and vision remain unaffected.

Orbital cellulitis lies behind that barrier and can threaten sight or spread deeper. Pain when moving the eye, restricted eye movement, reduced vision, double vision or a bulging eye raise concern for orbital involvement and change the urgency and treatment setting.

Start the prescribed antibiotic without delay, follow the dosing instructions, and attend the arranged review even if swelling begins to improve. Do not use leftover medicine or another person's eye drops; eye drops alone do not treat infection within the eyelid tissues.

Check vision and eye movement separately in each eye when the patient is awake and cooperative. Note whether redness is expanding or fever and pain are developing. Avoid squeezing a nearby spot, and keep hands clean before touching the eyelid.

Go to emergency care for pain with eye movement, difficulty moving the eye, blurred or double vision, a bulging eye, severe headache, vomiting, unusual drowsiness or confusion. These red flags are urgent regardless of whether the eyelid swelling seems mild.

Seek rapid reassessment if fever appears, swelling closes the eye, redness spreads, the prescribed medicine cannot be kept down, or symptoms worsen after treatment begins. Infants, people with reduced immunity and anyone without dependable follow-up need a lower threshold for hospital evaluation.

Confirm when the eye should be rechecked, which vision or movement changes demand emergency care, whether a sinus or skin source needs separate treatment, and whom to contact if the antibiotic cannot be taken.
