Antibiotics for pertussis: what patients and families should know

A clinician chooses the medicine according to age, pregnancy, allergies, other medicines, and how long the cough has been present. Treatment may not quickly stop established coughing fits because airway irritation can continue after the bacteria are no longer active.

Pertussis, also called whooping cough, is a bacterial respiratory infection. Antibiotics are most useful before severe coughing spells become established. Later treatment may still be considered to limit transmission or protect people who are particularly vulnerable, but it may have less effect on the cough itself.

Household members and other close contacts need individual assessment. A clinician or public-health team may recommend preventive antibiotics for some contacts, especially when an infant, a pregnant person near delivery, or someone at risk of severe illness could be exposed. Vaccination history should also be checked because antibiotics do not replace immunisation.

Take each dose exactly as prescribed and complete the course unless the prescriber changes the plan. Tell the clinician about medicine allergies, pregnancy, heart-rhythm problems, liver or kidney disease, and every prescription, supplement, or over-the-counter product being used. Do not share leftover antibiotics or use medicine prescribed for another person.

Reduce exposure to others according to the clinician's isolation advice, cover coughs, wash hands, and avoid close contact with babies or other vulnerable people until cleared. Small, frequent drinks can help with hydration. Arrange reassessment if doses are repeatedly vomited, the medicine cannot be kept down, or symptoms are worsening.

Seek emergency help for blue or grey lips, pauses in breathing, severe breathing difficulty, collapse, a seizure, marked confusion, or difficulty waking. Babies with suspected pertussis need prompt medical assessment because they may have breathing pauses without a typical whooping sound.

Get urgent advice for dehydration, repeated vomiting after coughing, chest pain, blood with coughing, or a clear deterioration. After an antibiotic dose, swelling of the lips or tongue, wheezing, throat tightness, or faintness can signal a severe allergic reaction and requires emergency care.

Ask how the diagnosis will be confirmed, whether the timing of symptoms changes the benefit of treatment, and how long close contact should be avoided. Confirm what to do after a missed or vomited dose, which side effects warrant a call, whether close contacts need assessment, and whether anyone in the household should update a pertussis-containing vaccine.
