Antibiotic anaphylaxis symptoms: what to recognise and do

Anaphylaxis is a rapidly developing allergic reaction that can affect breathing, circulation, skin, or the digestive system. Warning signs include noisy or difficult breathing, wheezing, a tight throat, hoarse voice, swelling of the lips or tongue, faintness, confusion, or collapse. Hives, flushing, vomiting, or abdominal cramps may occur as well, but a dangerous reaction can happen without a rash. Symptoms may begin while taking an antibiotic or after a dose.

Stop taking the suspected antibiotic and call the UAE ambulance service on 998 if breathing, throat swelling, severe dizziness, or collapse occurs. If an adrenaline auto-injector has been prescribed, use it promptly as instructed and still call for emergency help. Lie flat with the legs raised when possible; if breathing is difficult, sitting up may help. Do not stand or walk, and do not rely on an antihistamine to treat breathing or circulation problems.

Any sudden breathing difficulty, throat or tongue swelling, fainting, marked confusion, or signs affecting more than one body system after an antibiotic require emergency assessment. Symptoms can improve and then return, so medical observation is needed even after adrenaline seems to work. A mild isolated rash without breathing, swelling, faintness, or vomiting still deserves prompt clinical advice before another dose; seek urgent care if it spreads quickly or new symptoms appear.

Tell the clinician the antibiotic name, dose, timing, symptoms, and treatment received. Ask whether the reaction should be recorded as a suspected allergy, which related medicines require caution, and whether specialist allergy testing is appropriate.
