OTC anti-vomiting medication: a label-check guide

A pharmacist can help distinguish a motion-sickness product from one intended for stomach upset. That distinction matters because the wrong category may add side effects without addressing the pathway causing nausea.

OTC labels identify an active ingredient and the symptoms it is meant to relieve. Sedating antihistamines are commonly associated with travel sickness, while bismuth subsalicylate may be marketed for digestive upset. A single brand family can contain different ingredients in different products.

Warnings deserve the same attention as the dose. Some options are unsuitable with glaucoma, urinary retention, stomach bleeding risk, particular medicines, pregnancy or certain age groups. Local pharmacy availability can also differ.

Show the pharmacist a list or photo of everything you take, including cold remedies and herbal products. Explain whether vomiting followed travel, questionable food, diarrhoea, headache or abdominal pain. Check that two products do not duplicate an antihistamine or another active ingredient.

Use only the labelled amount and measuring device. Replace fluid with small repeated sips of oral rehydration solution. If drowsy, do not drive, operate machinery or mix the medicine with alcohol. Keep medicines in their original packaging and away from children.

Call for urgent help if vomiting occurs with a sudden severe headache, stiff neck, confusion, fainting, chest pain, severe abdominal tenderness, blood or coffee-ground material, or green fluid. A head injury followed by vomiting also needs prompt assessment.

Professional review is needed when you cannot retain liquids or develop very little urine, a persistently dry mouth, worsening dizziness or unusual lethargy. Recurrent vomiting, unexplained weight loss or symptoms that repeatedly return after the medicine wears off should not be managed by ongoing OTC use.

Ask what the active ingredient targets, whether it overlaps with another remedy, how sedation could affect work or driving, and how long self-care is reasonable. For a child, give the exact age and weight rather than choosing from an adult product.
