What does a tongue device do?
Often called a tongue-retaining device, it commonly uses a soft bulb or chamber that gently keeps the tongue in place. Unlike a mandibular advancement device, it does not depend mainly on moving the lower jaw forward.
Who might consider this design
The design may be discussed when the tongue contributes to airway narrowing or when a jaw-advancing mouthpiece is unsuitable. It may also be considered for someone whose teeth cannot support another oral appliance, although a clinician still needs to assess the mouth and airway.

Fitting and daily care
Arrange an oral and airway assessment before regular use. Follow the supplied method for placing the tongue; excessive suction can cause soreness. Begin only as advised, and check the tongue, gums and lips each morning for persistent redness, ulcers or swelling.
Clean the device after use with the method recommended by its manufacturer, rinse it thoroughly and allow it to dry before storage. Replace a device that splits, becomes rough or no longer holds its shape. Do not share it with another person.
Extra saliva, dry mouth, tongue tenderness or difficulty keeping the device in overnight may occur. A dental or sleep clinician can help determine whether adjustment or a different treatment is more suitable.
Symptoms that need attention
Stop using the device and seek prompt advice if it causes significant tongue swelling, bleeding, persistent ulcers, severe jaw pain or difficulty swallowing. Get urgent help if swelling affects breathing or if a loose or broken part may have been inhaled.
Regular loud snoring with witnessed pauses, gasping, morning headaches or pronounced daytime sleepiness calls for medical assessment. Obstructive sleep apnoea can present with these symptoms, and reduced snoring volume does not confirm that airway obstruction has resolved.
