Rabies vaccine injection site and local reactions

For adults and older children, the upper-arm deltoid muscle is a usual site for intramuscular rabies vaccine. For infants and small children, a clinician may select the outer thigh. The buttock is avoided for rabies vaccine because fat can prevent reliable delivery into muscle.

Rabies immunoglobulin is not another vaccine dose. When it is required, as much as practical is infiltrated into and around the bite or scratch, with any remainder administered according to the product protocol. Immunoglobulin and vaccine are not mixed in the same syringe or injected into the same anatomical site.

Some approved regimens use small injections into the skin rather than muscle. The treating service chooses the route from the product, local protocol and the person's clinical circumstances; patients should not select or change the route themselves.

Mild soreness, redness or swelling can occur where vaccine was given. Keep the area clean, move the arm gently, and use a cool compress wrapped in cloth if that feels soothing. Avoid rubbing or repeatedly pressing the injection site.

Before leaving the clinic, check where the vaccine and any immunoglobulin were administered and record the date, product details and next appointment. If you think a vaccine was placed in the buttock, or vaccine and immunoglobulin were put in the same site, contact the treating service promptly for review rather than repeating a dose yourself.

Call emergency services for trouble breathing, swelling of the face, lips or tongue, collapse, confusion, or a rapidly spreading raised rash after an injection. These can signal a severe allergic reaction and need immediate treatment.

Contact the clinic for redness that continues to spread, marked swelling, severe or worsening pain, pus, fever, arm weakness or persistent numbness. A local reaction does not automatically mean the rabies course should stop; the clinician can assess the symptom and advise how to continue protection safely.

Confirm the intended injection route and site, whether immunoglobulin was required, and how to manage the reaction you experienced. Mention bleeding disorders, immune-system conditions, medicines and previous vaccine allergies because they may affect administration or follow-up.
