Anti-rabies injection: when and why it is given

After a possible exposure, vaccination may be combined with careful wound cleaning and rabies immunoglobulin. The exact plan depends on the contact and whether the person was vaccinated previously.

Pre-exposure vaccination is considered for people whose work, activities, or travel create a relevant risk. It prepares the immune system but does not remove the need for medical care after a later exposure.

Post-exposure prophylaxis is urgent preventive care after infected saliva may have entered broken skin or the eyes, nose, or mouth. For a person who has not been vaccinated before, care can include vaccine plus rabies immunoglobulin placed around the wound when indicated. Someone previously vaccinated generally follows a different plan, which is why old vaccine records are useful.

Immediately wash every bite or scratch thoroughly with soap and running water. Rinse saliva from an eye, the nose, or the mouth with clean water. Wound cleaning is an essential part of prevention, even when you can reach a clinic quickly.

Seek a prompt professional risk assessment and describe the animal, its behaviour, the type of contact, the body site, and the country or area where it happened. Bring records of earlier rabies injections if available. If a vaccine series is started, follow the clinic’s schedule and contact the service if an appointment is missed rather than abandoning the remaining care.

Obtain medical advice promptly after an animal bite, a skin-breaking scratch that may contain saliva, or saliva contact with damaged skin or a mucous membrane. Treatment decisions should be made from the exposure history, not from waiting for symptoms.

Use urgent services for uncontrolled bleeding, a deep wound, facial or eye injury, severe pain, numbness, or difficulty moving the affected area. After an injection, seek emergency help for breathing difficulty, swelling of the face or throat, widespread hives, collapse, or severe dizziness. Mild soreness at the injection site can occur; a clinician can advise if it persists or concerns you.

Confirm whether you need vaccine alone or vaccine with rabies immunoglobulin, where each will be given, and when to return. Ask how pregnancy, an immune-system condition, current medicines, earlier rabies vaccination, or an allergy affects the plan. Request written documentation for any future clinician who continues the series.
