What to know about the anti-CCP blood test

Anti-CCP means antibodies to cyclic citrullinated peptides. The immune system can produce these antibodies in rheumatoid arthritis, an inflammatory condition that often affects joints on both sides of the body. A positive result supports that diagnosis when the symptom pattern and examination fit, while a negative result does not rule it out. The laboratory report may use its own reference range, so compare the value with the range printed on that report rather than a number found elsewhere.

The test uses a blood sample from a vein. Follow any preparation instructions supplied with the request, though the anti-CCP measurement itself does not usually require fasting. Tell the clinician about medicines, existing autoimmune conditions and any previous relevant results. At follow-up, describe which joints hurt or swell, whether stiffness is worse after waking, how long symptoms last and how they affect hand use, walking or work. Interpretation may also involve examination, inflammatory markers, rheumatoid factor and imaging.

Book a timely review for persistent joint swelling, prolonged morning stiffness or loss of joint function, regardless of the anti-CCP result. Seek urgent care for a joint that becomes suddenly very painful, hot and swollen, especially with fever or feeling acutely unwell, because infection and other urgent causes need prompt assessment. After the blood draw, obtain help if bleeding will not stop, swelling rapidly increases or the hand becomes numb or unusually pale.

Ask what the reported value and laboratory range mean alongside your symptoms, whether examination findings suggest inflammatory arthritis, and what further blood tests or imaging would change the assessment. Clarify whether referral to a rheumatology specialist is appropriate and what to do if swelling or stiffness progresses while waiting.
