The short answer
Anterior pelvic tilt describes the pelvis resting or moving in a forward-rotated position. Posture is usually assessed through history and physical examination. Imaging is considered when the clinician needs to examine internal structures or rule out another explanation for symptoms, not merely to label posture.
What MRI can and cannot answer
A pelvic MRI uses a magnetic field and radio waves to create detailed images of soft tissues, organs, joints, and bones within the scanned area. It can identify some structural problems that are not apparent during an external examination.
A scan taken while lying still does not fully show how the pelvis moves during standing, walking, lifting, or exercise. Image findings also need clinical context because a visible change may not be the source of pain. Depending on the symptoms, an examination of the hips, spine, abdomen, pelvic floor, strength, and movement pattern may be more informative initially.
Planning assessment and imaging
Describe where discomfort occurs, which movements trigger it, whether it travels into a leg or groin, and whether bladder, bowel, menstrual, sexual, or neurological symptoms are present. A clinician can then decide whether physiotherapy assessment, another test, or MRI addresses the actual concern.
Before MRI, tell the imaging team about pregnancy, implanted devices, metal fragments, previous surgery, kidney problems, allergies, or claustrophobia. Remove metal items as directed. If contrast is proposed, ask why it is needed and whether any preparation or follow-up applies.
Warning signs need faster review
Seek urgent medical assessment for new loss of bladder or bowel control, numbness around the genitals or buttocks, rapidly worsening leg weakness, severe pain after significant injury, fever with severe pelvic or back pain, or inability to bear weight. Sudden collapse or severe pain with heavy vaginal bleeding requires emergency care. These symptoms determine urgency regardless of whether pelvic tilt has previously been noted.
Questions before agreeing to MRI
Ask which diagnosis the scan is intended to investigate, whether the scan area includes the likely source of symptoms, and how the result would change management. Also check whether contrast is planned, who will explain the report, and what treatment is appropriate if imaging does not reveal a structural cause.
