Why Can Ankylosing Spondylitis Feel Worse at Night?

Short answer
Ankylosing spondylitis pain and stiffness may worsen at night because prolonged rest can aggravate inflammatory symptoms.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common night pattern
- Pain or stiffness after prolonged rest, sometimes disturbing sleep
- Helpful information to record
- Timing, pain location, awakenings, morning stiffness and response to movement
- Treatment review
- Recurring night symptoms may warrant reassessment of inflammation, pain relief and positioning
Why nights can be difficult
Discomfort may wake someone from sleep and can be followed by marked stiffness on getting up. Movement often eases this inflammatory pattern, though nighttime pain has other possible causes and is not enough on its own to diagnose ankylosing spondylitis.
Reading the overnight pattern
Inflamed joints and nearby tissues can feel stiffer after the body stays in one position. The pain may be felt deep in the lower back or buttocks, sometimes alternating sides, while inflammation around the ribs can make turning or taking a deep breath uncomfortable. A mattress or sleeping posture can add mechanical strain, so more than one factor may contribute.
The useful clues are timing and behaviour: when the discomfort starts, whether it wakes you, how long stiffness persists after rising, and whether gentle activity changes it. In a person with ankylosing spondylitis, recurring night symptoms can indicate that pain control, inflammation or the sleep setup needs review.
Steps for a more manageable night
Track sleep interruptions, pain location, morning stiffness and daytime activity, then share the pattern at your review. Follow the movement plan provided by your physiotherapist; gentle mobility before bed or after waking may be useful when it has been selected for you. During the day, alternate positions rather than remaining still for long periods.
Check whether your pillow and mattress support a comfortable, neutral position without forcing the neck or hips. Take prescribed medicine only as directed and discuss timing, missed relief or unwanted effects with the prescriber instead of changing the dose yourself. Persistent poor sleep also deserves attention because it can amplify pain and fatigue.
When nighttime pain needs medical review
Book a clinician review if night pain is new, steadily worsening, repeatedly waking you or no longer responding to the agreed treatment plan. Mention fever, unexplained illness, a hot swollen joint or new bowel, skin or eye symptoms because they change the assessment.
Seek urgent help for new limb weakness, numbness around the groin, loss of bladder or bowel control, severe chest pain or breathing difficulty. A painful red eye with light sensitivity or blurred vision requires urgent eye assessment. Severe spinal pain after a fall or impact should also be assessed promptly.
What to ask at follow-up
Ask whether your night symptoms suggest active inflammation, mechanical strain or both, and what changes to treatment or physiotherapy are appropriate. Clarify which exercises are suitable near bedtime, how medicine timing should be handled, and what symptom changes should bring your next appointment forward.

Questions people ask
Does pain at night prove that I have ankylosing spondylitis?
No. Night pain has several possible causes. Clinicians look at its pattern together with other symptoms, examination findings and any appropriate tests.
Why might moving around ease the discomfort?
Gentle movement can reduce stiffness that builds during rest and change pressure on sensitive tissues. The effect is a useful clinical clue but is not diagnostic by itself.
Should I exercise when pain wakes me?
Use only gentle movements already judged suitable for you, and stop if they sharply worsen symptoms or cause new weakness or numbness. Repeated awakenings should be discussed with your clinician or physiotherapist.
