The short answer
The thoracic spine runs through the mid-back and connects with the ribs. Inflammation around spinal joints and the places where ribs meet the spine or breastbone can make turning, reaching, coughing, or taking a deep breath uncomfortable.
Effects on the mid-back and ribs
Thoracic involvement may feel like a deep ache or stiffness between the shoulder blades, across the back, or around the chest wall. Ankylosing spondylitis can present with symptoms that are worse after rest and ease with movement. Muscle guarding can add soreness and make upright posture tiring.
Reduced rib movement can make deep breathing feel restricted even when the lungs themselves are not the source. However, chest or breathing symptoms cannot safely be attributed to the spine without assessment. Examination may look at spinal rotation, posture, rib expansion, shoulder movement, nerve function, and tenderness.
Maintaining thoracic movement
Break up long periods in one position and change posture through the day. A physiotherapist can teach thoracic rotation, extension, shoulder-girdle, and breathing exercises suited to your current mobility. Strengthening the upper back and trunk may support posture, while regular aerobic activity encourages fuller breathing.
Movement should be smooth and controlled; do not force a stiff spine or use aggressive manipulation. If a new exercise produces sharp chest pain, spreading numbness, dizziness, or breathlessness, stop and obtain advice. Discuss persistent night pain or a clear loss of movement with your treating clinician because inflammatory control may need review.
Chest and spine warning signs
Call emergency services for sudden or severe chest pressure, major breathing difficulty, fainting, coughing blood, or chest pain with sweating or nausea. These symptoms require urgent assessment even when rib or thoracic pain has happened before.
After a fall or impact, seek prompt care for new focal spinal pain, marked tenderness, weakness, numbness, or difficulty walking. A spine affected by ankylosing spondylitis may be more vulnerable to injury, and pain after trauma should not be managed solely with home exercises.
