Symptoms of ankylosing spondylitis

Short answer
Ankylosing spondylitis often presents with persistent lower-back or buttock pain and stiffness that improves with movement rather than rest.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Back pattern
- Symptoms may worsen after rest and ease with movement.
- Other sites
- Hips, shoulders, ribs, heels, eyes, and peripheral joints may be affected.
- Diagnosis
- The overall history, examination, and selected tests are considered together.
The short answer
Symptoms may be more noticeable overnight or on waking, and they can fluctuate. The condition can also affect the hips, shoulders, ribs, heels, eyes, or other joints. No single symptom confirms the diagnosis, because mechanical back problems and other inflammatory conditions can feel similar.
Patterns beyond back pain
Reduced spinal movement can make bending, turning, or looking over the shoulder harder. Rib-joint inflammation may make a deep breath uncomfortable, while inflammation where tendons attach to bone can cause heel or chest-wall tenderness. Fatigue may rise when inflammation is active.
Some people develop a suddenly painful, red, light-sensitive eye. Others have swollen peripheral joints, psoriasis, or inflammatory bowel symptoms. These features do not occur in everyone, but reporting them helps connect complaints that might otherwise seem unrelated.
What to record and do
Book a clinical review when back or buttock symptoms persist, recur, disturb sleep, or limit normal activity. Note when stiffness is strongest, what happens after rest and exercise, which joints are involved, and whether symptoms respond to prescribed medicine.
While awaiting assessment, stay gently active within comfort and vary prolonged sitting. If ankylosing spondylitis is diagnosed, care may combine medicine with physiotherapy, mobility exercises, strengthening, posture work, and aerobic activity. The programme should be adjusted during a flare and progressed as symptoms settle.
Red flags and rapid changes
A painful red eye, blurred vision, or marked sensitivity to light requires urgent eye assessment. New weakness, groin numbness, or loss of bladder or bowel control needs emergency care because these symptoms may indicate pressure on spinal nerves.
Get urgent help after a fall or impact if new neck or back pain develops, especially when the spine is already stiff. Fever with severe spinal pain, unexplained breathlessness, chest pain, or a rapidly worsening neurological symptom also warrants prompt evaluation.
Questions that sharpen the assessment
Ask which parts of your symptom pattern appear inflammatory, what alternatives are being considered, and whether blood tests or imaging would change the next step. If diagnosed, clarify how activity will be monitored, which exercises fit your mobility, and what signs should trigger a quicker review.

Questions people ask
Is morning stiffness the only symptom?
No. Ankylosing spondylitis can also present with buttock pain, night pain, fatigue, reduced movement, tendon-attachment pain, or inflammation outside the spine.
Can symptoms come and go?
Yes. Symptoms may flare and settle, while the pattern and functional impact can change over time.
Does back pain that improves with movement prove ankylosing spondylitis?
No. It is a useful inflammatory clue, but a clinician must consider the full pattern and other possible causes.
