How the shoulder can be affected
Shoulder pain in someone with ankylosing spondylitis may also arise from rotator-cuff irritation, the neck, posture, injury, or an unrelated joint problem. The location, onset, night pain, neck symptoms, and movements that reproduce discomfort help a clinician identify the likely source.
Clues in the pain pattern
Inflammatory shoulder involvement may produce deep aching, stiffness after inactivity, and restriction across several directions. Tendon or bursa irritation may hurt more through a particular arc or with reaching overhead. Pain travelling below the elbow with tingling, numbness, or neck-provoked symptoms can point toward nerve irritation.
Ankylosing spondylitis presents mainly in the spine and sacroiliac joints, but larger joints can be involved. Supporting the forearm on a pillow can make sleep more comfortable. Avoid forceful stretching into sharp pain or repeatedly testing a painful movement.
A physiotherapist can coordinate shoulder-blade control, rotator-cuff strengthening, chest mobility, and posture work with your spinal programme. If symptoms persist or the shoulder becomes increasingly stiff, arrange a review to assess inflammatory activity and other shoulder conditions. Medication changes should be made with the prescribing clinician.
When the shoulder needs prompt care
Seek urgent assessment if the shoulder is hot, red, markedly swollen, and accompanied by fever or feeling acutely unwell. After an injury, urgent care is appropriate for visible deformity, inability to move the arm, loss of sensation, or a hand that becomes pale, blue, or cold.
Call emergency services when shoulder discomfort occurs with chest pressure, severe breathlessness, faintness, sweating, nausea, or pain spreading to the jaw or arm. Those features may indicate a medical emergency rather than a musculoskeletal flare.
