Ankylosing spondylitis in the lumbar spine

How the structures involved in Ankylosing Spondylitis differ from normal

Short answer

Ankylosing spondylitis can inflame joints around the lumbar spine, causing lower-back pain, stiffness and gradually reduced movement.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Control inflammation and preserve mobility

The short answer

The condition often involves the sacroiliac joints where the spine meets the pelvis before or alongside the lumbar region. Symptoms may ease with movement and feel worse after rest, but patterns vary between people.

What lumbar involvement means

Ongoing inflammation can make bending, twisting and standing upright harder. Protective muscle tightening may add discomfort. Over time, healing can produce extra bone around spinal joints and ligaments, which may reduce flexibility; this does not happen in the same way or at the same pace for everyone.

Assessment considers the pain pattern, morning stiffness, mobility, posture and other features of inflammatory disease. Examination and, when appropriate, blood tests or imaging help separate inflammatory back pain from muscle strain, disc problems and other causes.

Protect movement and function

Keep active within tolerable limits rather than staying in bed for prolonged periods. A physiotherapist can tailor spinal mobility, hip flexibility, posture, strengthening and breathing exercises. Build activity gradually and adjust the load during a flare instead of forcing a painful end range.

Treatment may also include anti-inflammatory medicine or other disease-controlling treatment selected by a clinician. Mention stomach, kidney or heart conditions and all current medicines before using an anti-inflammatory drug. Track changes in sleep, morning mobility and everyday tasks so follow-up decisions reflect function as well as pain.

When lower-back symptoms need care

Arrange a clinical review if stiffness or pain is persisting, worsening, waking you repeatedly, or limiting work and self-care. New eye pain, eye redness or light sensitivity needs prompt assessment because eye inflammation can accompany ankylosing spondylitis.

Seek urgent care for new loss of bladder or bowel control, numbness around the groin, marked leg weakness, severe pain after a fall, chest pain or significant difficulty breathing. These features require assessment for problems beyond an ordinary flare.

Questions for your appointment

Ask which findings suggest active inflammation, whether the sacroiliac joints are involved, and what movement goals fit your current mobility. It is also useful to clarify how to modify exercise during a flare, which medicine effects to report, and when follow-up imaging or specialist review would change management.

Questions people ask

Does lumbar pain mean the spine has fused?

No. Pain and stiffness can come from active inflammation or muscle guarding without spinal fusion. Examination and imaging, when clinically appropriate, help show structural change.

Should I exercise when my lower back is stiff?

Gentle, controlled movement is often useful, but the programme should match your symptoms and function. Reduce intensity during a flare and stop an activity that causes sharp or escalating pain.
