The short answer

Its inflammation is driven by abnormal immune activity and commonly targets the sacroiliac joints, spinal joints, and ligament or tendon attachment sites. The label does not mean the immune system is weak, and the condition is not contagious.

What immune-mediated inflammation means

Immune signals that normally help control injury or infection remain active in tissues without a helpful purpose. This can cause pain and stiffness after rest, reduced spinal movement, and inflammation in hips, knees, heels, or other sites. It may also occur alongside inflammation in the eyes, skin, or bowel.

There is no single blood result that confirms ankylosing spondylitis. Assessment combines the symptom pattern, physical examination, imaging, family history, and selected blood tests. A gene marker can support the assessment in some people, but having the marker alone does not establish the diagnosis.

How inflammation is managed

Treatment aims to reduce inflammatory activity, preserve mobility, and support daily function. Depending on disease activity and health history, a rheumatologist may recommend anti-inflammatory medicine or a treatment that blocks a specific immune pathway. These medicines differ from general immune suppression and require individual screening and monitoring.

Regular exercise and physiotherapy complement medication by working on spinal mobility, posture, strength, and breathing movement. During a flare, the plan may need lower-impact activity or shorter sessions rather than complete bed rest. Do not stop prescribed immune-targeting medicine suddenly without discussing the reason with the treating team.

Symptoms that need medical attention

Contact your clinician if pain, stiffness, fatigue, or joint swelling is increasing despite the current plan, or if treatment causes fever, a persistent cough, a new rash, or other possible infection symptoms. The response may involve checking disease activity, infection, medicine effects, or a different cause of pain.

A painful red eye with light sensitivity or blurred vision needs urgent assessment. New leg weakness, numbness around the groin, or loss of bladder or bowel control also requires emergency evaluation.
