The short answer

How neck involvement may feel
Inflammation around spinal joints and nearby ligament attachments may produce aching at the base of the skull or across the neck and shoulders. Stiffness can be more noticeable after sleep or prolonged sitting and may ease as the body warms up.
Over time, some people with ankylosing spondylitis lose neck range, which can make checking traffic, looking upward or finding a comfortable pillow position harder. Move with control
Ask a physiotherapist to measure neck and upper-back movement and observe posture. Gentle active movements, upper-back mobility, shoulder-blade control and breathing work may be included according to your examination. Avoid high-velocity neck manipulation, particularly if movement is already restricted or the spine has structural changes.
Adjust screens so the head is not held down for long periods, change position regularly, and use vehicle mirrors or other aids if turning is limited. Do not drive when you cannot scan the road safely.
Neck exercises support function but do not replace treatment of active inflammation. Tell your rheumatology clinician about increasing stiffness or night pain so the overall plan can be reviewed.
Neck warning signs
Seek emergency assessment after a fall, collision or sudden neck injury if pain is substantial, even when the injury seemed minor. A stiff or structurally changed spine may be more vulnerable, so keep the neck still and avoid trying to test its range yourself.
Urgent help is also needed for new arm or leg weakness, spreading numbness, difficulty walking, loss of bladder or bowel control, severe headache after injury, or breathing or swallowing difficulty. Fever with severe neck stiffness and feeling acutely unwell also warrants urgent care.
