Sex does not determine one fixed course. Early assessment matters when pain repeatedly wakes you, improves with movement rather than rest, or is accompanied by stiffness that makes mornings difficult.

Recognising the broader pattern

Ankylosing spondylitis belongs to a group called axial spondyloarthritis. Inflammation often begins around the joints between the pelvis and lower spine, producing deep buttock or low-back pain. Some men also develop heel pain, swelling in a limb joint, chest-wall discomfort or fatigue.

A painful red eye with light sensitivity can reflect eye inflammation associated with the condition. Skin psoriasis or ongoing bowel symptoms can also be relevant, so mention them even when the appointment is focused on the back.

Diagnosis is based on the whole clinical picture. A clinician considers the symptom pattern, examination, blood tests and imaging where appropriate; no single symptom by itself establishes ankylosing spondylitis.

Protect function over time

Schedule a medical review for persistent inflammatory-pattern back pain. Beforehand, note when symptoms began, whether pain alternates between the buttocks, what happens after rest or exercise, and whether relatives have related inflammatory conditions.

Treatment is matched to current inflammation and functional needs. It can combine medication with physiotherapy, mobility work, strengthening, posture practice and breathing exercises. If work includes prolonged sitting, driving or heavy lifting, discuss practical task changes instead of pushing through a repeated flare.

Smoking can worsen respiratory and cardiovascular health, which already deserve attention when movement is restricted, so ask for cessation support if needed.

Symptoms that cannot wait

Arrange same-day eye care for a painful red eye, blurred vision or strong light sensitivity. Seek emergency help for new limb weakness, numbness around the groin, loss of bladder or bowel control, severe breathing difficulty, chest pressure, or substantial spinal pain after trauma.

Ask for an earlier routine review when night pain increases, movement is progressively declining, a joint stays swollen, or treatment side effects make you skip doses.
