The short answer

Ankylosing spondylitis is an inflammatory condition that mainly affects the spine and sacroiliac joints. Fibromyalgia changes how the nervous system processes pain and is associated with widespread tenderness, fatigue and unrefreshing sleep. Having both can make symptoms harder to interpret, so assessment should look beyond pain intensity alone.

How the symptom patterns differ

Active ankylosing spondylitis can present with inflammatory back or buttock pain, morning stiffness and reduced spinal movement. Fibromyalgia more often produces pain across several body areas, sensitivity to touch, poor-quality sleep, fatigue and difficulty concentrating. One person may experience features of both at the same time.

Tenderness and severe pain do not by themselves show that joint inflammation is active. A clinician may consider the location and timing of symptoms, physical examination, sleep, function, medication response and, when appropriate, tests or imaging. This distinction matters because increasing anti-inflammatory treatment may not address pain amplification from fibromyalgia.

Build a plan for both conditions

Keep a brief symptom record covering morning stiffness, widespread tenderness, sleep, fatigue and activities that trigger or ease discomfort. Take it to your rheumatology review so the plan can address inflammation, pain processing and daily function separately.

Regular, paced movement can support spinal mobility without provoking a prolonged flare of widespread pain. A physiotherapist can help set a manageable starting level and progress strengthening, aerobic activity and flexibility gradually. Sleep routines, stress management and treatment of mood symptoms may also reduce the overall symptom burden. Use anti-inflammatory or other medicines only as prescribed, and report side effects or limited benefit rather than changing doses yourself.

When symptoms need prompt review

Arrange a medical review if pain changes substantially, stiffness keeps worsening, sleep or fatigue prevents normal activities, or the current plan no longer helps. Seek urgent assessment for a painful red eye with light sensitivity or blurred vision, new weakness or numbness, loss of bladder or bowel control, chest pain, severe breathing difficulty, or fever with a hot and swollen joint.

Ask which findings suggest inflammation and which fit fibromyalgia, how progress will be measured, and what level of activity is appropriate during a symptom flare. You can also ask whether sleep problems, medication effects or another condition could be adding to fatigue and pain.
