Ear pain with ankylosing spondylitis

Having ankylosing spondylitis does not make every new symptom part of the condition. The location, triggers, hearing changes, jaw movement, and examination help identify where the pain begins.

Pain felt in the ear may start in the ear canal or middle ear, but it can also be referred from the jaw joint, teeth, throat, or upper neck. Jaw-joint irritation may be more noticeable with chewing, clenching, or clicking. Neck-related pain may change with head position and accompany stiffness or restricted movement.

Ankylosing spondylitis mainly involves the spine and sacroiliac joints, although inflammation can occur elsewhere. A clinician should avoid assuming a connection before checking common local causes, medication history, recent infection, and any skin changes around the ear.

Note whether the pain is inside or around the ear, whether it follows chewing or neck movement, and whether there is discharge, fever, reduced hearing, dizziness, tooth sensitivity, or a sore throat. This pattern can direct you toward primary care, dental assessment, or an ear specialist.

Avoid placing cotton buds, drops, or objects into the ear unless a clinician has advised the specific product. Continue prescribed ankylosing spondylitis medicines as directed, and check before adding pain relief because interactions and personal health conditions can affect what is suitable.

Arrange prompt assessment for severe or worsening pain, discharge, fever, new hearing loss, marked dizziness, swelling behind the ear, or a rash with blisters around the ear or face. Seek emergency help for facial weakness, confusion, severe headache with neck stiffness, or sudden major hearing loss.

If you take medicine that reduces immune activity, report fever or suspected infection promptly to the prescribing team. Do not stop such medicine abruptly unless your treating clinician or urgent-care professional tells you to do so.

Ask whether the ear itself looks inflamed, whether the jaw, teeth, throat, or neck could be referring pain, and whether your medicines change the infection plan. Also ask which symptom should trigger a same-day review if the initial examination is reassuring.
