Discomfort may be felt around the breastbone, sides of the chest, or back and can become noticeable with a deep breath. Chest tightness has many possible causes, so it should not automatically be attributed to the spine or rib joints.
Why the chest wall may feel restricted
The ribs connect to the spine and breastbone through joints that move during breathing. Inflammation at these joints or nearby attachment sites can make deep breathing, coughing, twisting, or reaching uncomfortable. Upper-back stiffness can further reduce the sense of chest expansion.
A clinician may ask whether the sensation changes with movement, pressure, exertion, meals, or breathing. Examination can include the chest wall, lungs, heart, spinal mobility, and oxygen status, depending on the symptoms. This distinction matters because heart, lung, digestive, and anxiety-related problems can also feel like tightness.
Steps that may support chest mobility
If serious causes have been excluded, follow the treatment plan for inflammatory disease activity. A physiotherapist may teach controlled deep-breathing, rib mobility, upper-back extension, and posture exercises. These should feel manageable; stop if they trigger dizziness, marked breathlessness, or increasing chest pain.
Break up long periods of sitting and keep regular whole-body activity within your tolerance. Note whether symptoms follow exertion or occur at rest, how breathing affects them, and whether palpitations, cough, fever, or digestive symptoms occur at the same time.
When chest tightness is an emergency
Call emergency services for new, severe, or unexplained chest pressure or tightness, especially with breathlessness, sweating, faintness, nausea, palpitations, or pain spreading to the arm, jaw, shoulder, or back. Do not drive yourself if you feel seriously unwell.
Prompt assessment is also needed for sudden breathing difficulty, coughing blood, blue or grey lips, or chest pain after an injury. Contact your usual clinician soon when milder tightness keeps returning, limits breathing or activity, or differs from your familiar musculoskeletal symptoms.
