Can trauma cause ankylosing spondylitis?

Short answer
Physical trauma is not considered the underlying cause of ankylosing spondylitis, although an injury can bring existing symptoms to attention.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

The short answer
Ankylosing spondylitis is an immune-mediated inflammatory condition influenced by genetic susceptibility. A fall, collision, or lifting injury can damage tissues or aggravate pain, but it does not create that underlying immune process.
Why symptoms may appear after an injury
Pain after trauma may lead to examination or imaging that uncovers previously unrecognised sacroiliac or spinal inflammation. Reduced activity during recovery can also make established stiffness more noticeable. Timing alone does not prove that trauma caused the condition.
Mechanical injury pain often relates closely to a damaged structure and particular movements. Inflammatory back pain may be more troublesome after rest, include marked morning stiffness, disturb sleep, and ease with movement. The patterns can overlap, so a clinical assessment matters.
Separate injury care from inflammatory care
Describe the accident, immediate symptoms, and how the pain pattern changed afterward. Tell the clinician about back or buttock stiffness that existed before the event, previous episodes, eye inflammation, psoriasis, bowel symptoms, and relevant family history.
Follow injury precautions while the diagnosis is clarified. A physiotherapist can modify movement without forcing a painful area and can later rebuild mobility and strength. If ankylosing spondylitis is suspected, rheumatology assessment may include examination, blood tests, and appropriate imaging.
When trauma symptoms are urgent
Seek emergency care after an injury if there is new limb weakness, spreading numbness, loss of bladder or bowel control, numbness around the groin, breathing difficulty, confusion, or severe pain with inability to stand or move safely.
Arrange a prompt review when pain is worsening rather than settling, fever accompanies back pain, or persistent night pain and morning stiffness continue beyond the expected injury recovery. These features warrant assessment; they do not on their own diagnose ankylosing spondylitis.
Questions to clarify the diagnosis
Ask which findings fit the injury and which suggest inflammation, whether earlier symptoms change the interpretation, what activity restrictions are temporary, and when reassessment is needed. Clarify whether imaging is intended to examine trauma, sacroiliac inflammation, or both.

Questions people ask
Can a car accident trigger ankylosing spondylitis?
An accident is not considered the underlying cause. It may worsen pain, alter activity, or prompt tests that reveal inflammation which was already developing.
How can a clinician tell injury pain from ankylosing spondylitis?
The clinician considers the timing, pain behaviour, morning stiffness, earlier symptoms, examination, and relevant tests. No single symptom or scan detail should be interpreted in isolation.
