Ankylosing spondylitis and the TMJ

Short answer
Ankylosing spondylitis can involve the temporomandibular joint, but jaw pain may also come from muscle tension, dental problems or another joint disorder.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- TMJ
- The joint connecting the lower jaw and skull
- Possible symptoms
- Pain, stiffness, tenderness, locking or limited opening
- Assessment
- May involve medical, dental and physiotherapy input
The short answer
The temporomandibular joints, often shortened to TMJs, connect the lower jaw to the skull. Pain, stiffness or limited opening deserves assessment when it persists or interferes with eating, speaking or oral care.
Understanding jaw symptoms
Inflammation related to ankylosing spondylitis may present with jaw-joint tenderness, stiffness or reduced movement. Clicking alone can occur without inflammatory disease, while clenching, tooth problems and irritation of the chewing muscles can produce similar discomfort.
A clinician or dentist can examine jaw opening, movement symmetry, teeth and chewing muscles. They will also consider whether symptoms follow the broader inflammatory pattern, such as morning stiffness or swelling elsewhere, before deciding whether imaging or specialist input is useful.
Reducing strain while seeking care
Choose softer foods temporarily if chewing increases pain, take smaller bites and avoid gum, nail biting or repeatedly opening the mouth wide. Keep the jaw relaxed with the teeth apart when resting. Heat or cold may be used briefly according to comfort, with a barrier protecting the skin.
Do not force jaw stretches or copy exercises that have not been assessed for your problem. A physiotherapist with jaw experience may guide gentle movement, posture and muscle control, while a dentist checks for dental causes or an appropriate oral appliance. Inflammatory treatment remains the responsibility of the medical team.
When jaw symptoms need care
Arrange prompt assessment if the jaw repeatedly locks, opening becomes progressively restricted, eating is difficult or symptoms continue despite reducing strain. Fever, facial swelling, a bad taste or severe dental pain needs urgent dental or medical review because infection is possible.
Seek emergency care after facial injury if the bite suddenly changes, the jaw appears displaced, bleeding will not stop, breathing is difficult or swallowing becomes unsafe. These signs require immediate assessment rather than home exercises.
Questions for a jaw assessment
Ask whether the joint, chewing muscles or teeth appear to be the main source, and whether current ankylosing spondylitis activity could be contributing. Clarify which foods and movements to limit, which exercises are suitable and who should review worsening restriction.

Questions people ask
Does jaw clicking mean ankylosing spondylitis is affecting the TMJ?
No. Clicking can have several explanations and is not enough to identify inflammation. Pain, restriction and the wider symptom pattern provide more useful context.
Can I massage a painful jaw?
Gentle work around tense chewing muscles may help some people, but avoid force directly over a painful or swollen joint and seek assessment if symptoms persist.
Who should assess jaw pain first?
A dentist can check teeth and jaw mechanics, while your medical team can assess inflammatory activity. The most suitable starting point depends on accompanying symptoms.
