What the swelling may indicate
The amount of swelling alone cannot identify the injury. Where it hurts, whether you can bear weight, visible deformity, bruising and tenderness directly over bone help determine whether an examination or imaging is needed.
Possible injuries after a fall
A twist during the fall can stretch ankle ligaments, while a direct impact can bruise tissue or bone. A fracture may present with swelling, bruising, focal bone tenderness, difficulty taking steps or an ankle that looks out of position. Some fractures still allow limited walking, so weight-bearing does not rule one out.

Protect the ankle
Stop the activity and avoid forcing weight through the leg. Rest the ankle in a comfortable position, elevate it, and apply a wrapped cold pack for short periods; never place ice directly on the skin. Remove an anklet or tight footwear before swelling increases.
If the ankle is not deformed and movement is tolerable, gentle toe and ankle motion can limit stiffness. Do not test the injury by repeatedly hopping or walking on it. A pharmacist or clinician can help choose pain relief that is compatible with your health conditions and other medicines.
Signs that need assessment
Seek emergency care if the ankle is visibly deformed, a bone is exposed, bleeding is heavy, or the foot becomes cold, pale, blue, numb or weak. Keep the limb still and do not try to straighten a deformed joint.
Obtain prompt assessment for severe or worsening pain, inability to take steps, marked tenderness over a bone, rapidly increasing swelling, or a wound over the injured area. Arrange follow-up if pain and function are not steadily improving, even if initial home care seemed helpful.
