What should you know about ankle plaster cast?

The short answer
Keep the cast dry, do not place objects inside it, and follow the exact weight-bearing instructions from the treating team. New numbness, discoloured toes, severe pressure or pain that continues to worsen despite elevation can signal a cast problem and needs urgent assessment.
What the cast is doing
Plaster moulds closely around the leg and ankle, limiting movement so a fracture or selected soft-tissue injury can remain aligned. Padding protects the skin beneath it. A newly applied cast may be split or left partly open to accommodate early swelling, then replaced or completed when swelling settles.
Some casts permit weight through the foot, while others must remain completely unloaded. The injury pattern, stability and cast design determine this; comfort alone does not show that weight bearing is safe. Crutches or another walking aid should be fitted and used as demonstrated.
Daily cast care
Elevate the ankle so the toes are above the level of the heart when swelling builds, particularly soon after application. Move the toes regularly if permitted and check their colour, warmth, sensation and ability to move. Keep the cast away from water, sand, powders and lotions, and protect it during washing without immersing it.
Never trim the cast or push a ruler, pen or other object inside to scratch. Report cracks, soft areas, wet padding, a loose fit, rough edges, bad odour, discharge or persistent rubbing. Attend scheduled fracture-clinic checks because repeat examination or imaging may show whether alignment is maintained and when the cast can change or come off.
Cast warning signs
Seek urgent help if the toes become blue, pale or cold; numbness or pins and needles persists; toe movement becomes difficult; pain rapidly intensifies; or the cast feels increasingly tight despite elevation. These symptoms may reflect impaired circulation, nerve pressure or dangerous swelling.
Prompt review is also needed for burning pain at one spot, fluid or blood soaking through, a foul smell with fever, a cast that becomes wet through, or a cast that breaks and no longer supports the ankle. Do not wait for the next routine visit when these changes appear.

Can I walk on an ankle plaster cast?
Only if the treating team specifically says the cast is weight bearing and explains how much load is allowed. Walking on a cast that is not designed for it can disturb the injury or damage the cast.
What should I do if the skin itches beneath the cast?
Keep the skin cool and avoid inserting anything beneath the cast, as scratches can become infected without being visible. Contact the cast team if itching is intense, localised, associated with burning or accompanied by odour or discharge.
Why does a cast sometimes become loose?
