Can ankylosing spondylitis cause ankle pain?

Pain may occur with swelling, warmth, stiffness, or tenderness around the heel and Achilles tendon. Ankle symptoms can accompany back or hip stiffness, but a new painful ankle can also have another cause, such as an injury, gout, or infection.

What the ankle symptoms may mean

Ankylosing spondylitis is an inflammatory condition that mainly affects the spine but can also involve joints in the legs. Inflammation at a tendon or ligament attachment is called enthesitis; around the ankle, this often causes focused tenderness behind or beneath the heel. Inflammation inside the ankle joint may produce deeper pain, swelling, and reduced movement.

Symptoms that are stiff after rest and loosen with gentle activity can fit an inflammatory pattern. Their timing, location, and relationship to an existing diagnosis help a clinician interpret them; ankle pain alone does not establish ankylosing spondylitis.

What you can do now

Reduce activities that sharply increase pain, while keeping the ankle moving gently within a comfortable range. Supportive footwear and a cool pack wrapped in cloth may help a swollen or tender area. Avoid forceful stretching of a painful Achilles tendon or heel attachment.

Record when the pain is worst, whether swelling is present, and whether back stiffness or other joints are flaring. A rheumatology or physiotherapy review can distinguish joint inflammation from enthesitis and adjust exercise accordingly. Use anti-inflammatory medicine only if it is suitable for you and follow the prescribed or labelled directions.

Arrange a clinical review if ankle pain persists, repeatedly returns, limits walking, or comes with visible swelling or worsening morning stiffness. Contact your treating clinician sooner if symptoms break through your current ankylosing spondylitis plan or a medicine causes concerning effects.

Seek urgent care for a hot, red, rapidly swollen ankle, especially with fever or feeling unwell. Urgent assessment is also appropriate after a significant injury, if you cannot bear weight, or if the foot becomes numb, unusually pale, blue, or cold.
