The absence of swelling does not show how serious the problem is. How the pain began, its exact location and whether you can walk normally offer more useful clues.
What pain without swelling can mean
Pain after a longer walk, new exercise or repeated loading may come from irritated tendons, muscles or the joint. Pain after a twist can occur with an ankle sprain even when swelling is slight or absent. A fracture can also present with focal tenderness and difficulty bearing weight, so appearance alone cannot exclude one.
Notice whether the discomfort is at the inner ankle, outer ankle, front, back or directly over a bone. Also track stiffness, clicking, instability, numbness and whether symptoms occur at rest or only with movement.
What to do now
Reduce activities that reproduce the pain and choose comfortable, supportive footwear. Gentle ankle movement within a comfortable range can limit stiffness; stop if it causes sharp pain. A wrapped cold pack may ease soreness after activity, with cloth protecting the skin.
Arrange a clinical review if pain keeps returning, affects walking or is not settling with reduced load. A clinician may examine tender structures, ankle stability, circulation and nerve function, then decide whether imaging or rehabilitation is appropriate.
When to seek care promptly
Seek urgent assessment after an injury if you cannot take steps, the ankle looks misshapen, pain is intense, or one spot over a bone is very tender. Get immediate help for a cold or pale foot, new numbness, marked weakness, or rapidly worsening pain.
Redness, warmth, fever, an open wound or new swelling also warrants timely medical advice, even if swelling was absent at first.

Can an ankle sprain occur without swelling?
Yes. An ankle sprain may cause pain, tenderness or a sense of instability with little visible swelling, particularly when the tissue injury is mild.
Can I keep exercising if my ankle hurts but is not swollen?
Avoid movements that reproduce pain or change your gait. Lower-impact activity may be reasonable if it stays comfortable during and after activity, but worsening pain needs assessment.
Does no swelling rule out a fracture?
