Angular cheilitis and the upper lip

A corner-based fissure may look more prominent on the upper limb of the mouth angle, especially where a deep fold traps saliva. It may sting when you smile, yawn or eat. Changes can occur on one side or at both corners.

Inflammation covering the centre of the upper lip, the skin beneath the nose or the full lip border is broader than the expected angular pattern. Irritant or allergic contact, lip-licking dermatitis, infection and sun-related lip damage are among the alternatives a clinician may assess.

Blot saliva rather than wiping repeatedly, dry the fold after meals and protect it with a thin layer of bland fragrance-free ointment. Stop any new lipstick, balm, toothpaste or facial product that touches the site until the trigger is clearer. Avoid licking and do not pull off scale or crust.

If infection or marked inflammation is suspected, treatment may include an antifungal, antibacterial medicine or a carefully selected topical corticosteroid. Because these options are not interchangeable, use medicated creams only with advice about the diagnosis and application site.

Seek urgent help for swelling that affects breathing or swallowing. Promptly report fever, pus, rapidly advancing redness, clusters of blisters or pain that is becoming intense. Have the area examined if it persists, frequently recurs, forms a hard spot, bleeds without being picked, or sits mainly on the central upper lip rather than at the corner.

Ask whether the distribution truly fits angular cheilitis and whether a swab, dental assessment or contact-allergy evaluation is useful. Clarify whether saliva pooling, denture fit, dry mouth or a skin condition is contributing. Request written instructions for the amount, location and sequence of any barrier and prescription products.
