Angular cheilitis on the top lip

The word angular refers to the mouth angles. A split beginning at a corner can extend slightly onto the upper lip and produce soreness, scale or a small crust. Changes centred beneath the nose or spanning the lip border fit less neatly with angular cheilitis.

Wider top-lip irritation may follow lip licking, cosmetics, toothpaste, shaving products, sun exposure or contact allergy. Blisters, honey-coloured crust, a firm lump or a sore that does not heal require a clinician to consider other diagnoses.

Pause newly introduced lip and facial products, wash with lukewarm water, pat rather than rub, and use a simple fragrance-free ointment. Keep saliva off the corner and avoid stretching the split while eating. Emollient therapy can support the barrier, while prescription treatment depends on whether inflammation or infection is present.

Get urgent help if lip swelling comes with tongue swelling, breathing trouble or difficulty swallowing. Obtain prompt care for rapidly spreading redness, fever, pus, severe pain or clusters of painful blisters. Book a dermatology or primary-care visit when the top-lip change persists, repeatedly returns, bleeds without clear injury or remains confined away from the corners.

Describe where the change began and bring the lip products, toothpaste and facial treatments you use. Ask whether the pattern suggests contact dermatitis, infection or another type of cheilitis, and whether patch testing or a swab would change management. Confirm exactly where and how long to apply any prescribed medicine.
