Angular cheilitis and swollen lips: what to know

Angular cheilitis is inflammation where the upper and lower lips meet. It can cause redness, splitting, crusting, tenderness or burning, sometimes on both sides. Local inflammation can make the corners look mildly puffy, but swelling across much of one or both lips is not the typical pattern.

Saliva collecting in the folds can irritate the skin and allow yeast or bacteria to contribute. Dryness, lip licking, dentures that alter the mouth folds, and nutritional or health problems can also matter. Angioedema often presents with deeper, sudden swelling of the lips and may occur with swelling elsewhere or breathing symptoms.

Gently wash the mouth corners, pat them dry and avoid licking, picking or peeling the cracks. A plain, fragrance-free barrier ointment can shield irritated skin from saliva. Pause new lip products if symptoms began after using them, and avoid sharing towels or lip products while the skin is broken.

Arrange a clinical assessment if the area keeps returning, spreads, forms pus, or does not settle with simple care. Treatment depends on the cause and may include an antifungal, an antibiotic, treatment for irritation, denture adjustment, or checks for an underlying health issue. Do not apply leftover steroid or combination creams because they can mask infection or worsen some causes.

Seek emergency help for sudden lip swelling with tongue or throat swelling, trouble breathing, wheezing, faintness, difficulty swallowing, or a rapidly changing voice. These features can signal a serious allergic reaction or airway swelling.

Get prompt medical advice for rapidly increasing swelling, fever, severe pain, spreading facial redness, eye involvement, or inability to drink. Even without these signs, new swelling after a medicine, food, sting or cosmetic product deserves timely assessment, particularly if it has happened before.

Ask whether the cracking looks irritant, fungal, bacterial or contact-related; whether a swab or health check would change treatment; which barrier product is suitable; and which medicine or product exposures could explain the broader swelling. Bring a list of medicines and lip or dental products, plus photographs if swelling comes and goes.
