Severe angular cheilitis

Short answer
Severe angular cheilitis involves deep or extensive inflammation, marked pain or infection features and should be medically assessed.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Severe features
- Deep cracking, marked pain, swelling, discharge or spread
- Assessment goal
- Identify infection, irritation and contributing factors
- Emergency sign
- Breathing or swallowing affected by swelling
What makes angular cheilitis severe?
Concerning changes include fissures that repeatedly bleed, substantial swelling, pus, spreading crust or pain that interferes with eating and drinking.
What a severe presentation may reflect
Extensive damage may follow ongoing saliva exposure and friction, secondary yeast or bacterial growth, a strong irritant reaction, or more than one factor. A clinician also needs to exclude cold sores, contact dermatitis and other lip or mouth conditions that can resemble angular cheilitis.
Repeated or difficult-to-heal episodes may prompt review of dentures or bite, dry mouth, skin disease, medicines, nutrition and health conditions that affect healing or infection risk.
Protect the area before you are seen
Keep the corners clean, pat rather than rub, and use a bland fragrance-free barrier if it does not worsen discomfort. Sip fluids and choose soft, non-stinging foods if opening the mouth is difficult. Leave crusts and blisters intact.
Do not delay assessment while cycling through antifungal, antibiotic and steroid creams. Severe inflammation may need a diagnosis-specific plan, and some medicines can aggravate an infection or trigger further irritation.
Choose the right level of care
Seek same-day medical advice for rapidly worsening pain, expanding redness, significant swelling, discharge, fever, or inability to eat and drink adequately. People with reduced immunity or poorly healing skin should arrange prompt review rather than relying on self-care.
Call for emergency help if facial or mouth swelling makes breathing or swallowing difficult, or if you become confused, faint or severely unwell. These are escalation signs regardless of whether the original crack seemed minor.
What to cover during assessment
Tell the clinician how quickly the area changed, whether fluid intake is affected, which products were applied and whether episodes recur. Ask what diagnosis is being treated, how to use each medicine and which change should trigger reassessment.

Questions people ask
Can severe angular cheilitis be treated at home?
Barrier care may protect the skin while you arrange help, but severe or spreading changes need medical assessment and targeted treatment.
Why might a clinician examine my mouth or dentures?
Oral changes, appliance fit and saliva pooling can help explain ongoing irritation at the corners.
Should I use a strong steroid for severe redness?
Do not start one without clinical advice, because severe redness may include infection or represent a different condition.
